Program or Service Description: Specialized care and support to persons living with cancer or ALS.

Home nursing visits are arranged in collaboration with patients and families to allow for assessments, teaching, symptom management and active listening.

The NOVA team helps by providing support and comfort to both the patient and the family during this difficult time. This enables the patient to remain at home in familiar surroundings for as long as is possible or desired.
Age group: 18-34 (young adult), 35-64 (adult), 65+(senior)
Eligibility requirements: Adults living with cancer or ALS
Schedule: Nurses on call 24/7
